The main problem regarding the implementation of the hospital management model “is to ensure that doctors have the information and incentives to make cost-effective decisions at each level and to meet the expectations of patients. 
As the article points out, “ the lack of incentives by the system itself for its use, the suspicion of the unreliability of the results, the technical difficulty of the studies requires minimal training to interpret the results and to identify the quality of the same.  Furthermore, the difficulty of moving the allocations of the different budget items, in many cases makes it impossible to apply the information acquired. And all this, despite moving within a framework in which there is a strong interest in accessing information about the effectiveness and safety of health technologies. ”
Therefore, there is a need for real involvement on the part of all organizations so that the implementation of clinical management systems goes ahead. Also a greater training of all the agents involved to achieve the greatest possible effectiveness and efficiency.
